NQT induction assessment

When filling in the induction assessment forms please make sure:

on the forms for the end of the first and second periods (core standards), you indicate which period the form is for
the NQT's personal details are accurate and spelt correctly
the NQT's DCSF number is recorded accurately
the appropriate body receiving the report is Ealing Council
the date of the beginning of this assessment period in this institution is the start date of induction
the recommendations boxes are marked clearly
under 'does the NQT work...' you record whether the NQT works full-time or part-time

Once the form is completed, the induction tutor should ask the NQT to read through it and make any comments.

Before sending the form to the local authority, please ensure the last page has been signed by the NQT, induction tutor and headteacher.

Deadline dates 2011/12

Autum 2011: Friday 9 December 2011
Spring Term 2012: Friday 30 March 2012
Summer Term 2012: Friday 13 July 2012
