Manager referral forms
Managers wishing to refer an employee to the occupational health unit must complete a referral form:
This authorisation form/referral form (word) has been designed to allow headteachers/ managers to refer employees to occupational health electronically. Guidance (word) on filling in the form is also available.
How does the form work?

download the form and save in a suitable place on your PC (use the 'save as' option if you want to give it a different filename)

use the tab key or cursor to move between fields (some areas of the form are protected and will not allow you to change them)

ensure you select the relevant boxes from the pre-set options

remember to save the form as you go along

when you have finished save the form to your PC and to disc (in case of corruption or system failure)

email the form as an attachment to wardc@ealing.gov.uk or wilkinsSa@ealing.gov.uk

remember to mark the email 'confidential', to ensure that employee confidentiality is maintained
It is your responsibility to ensure that the manager's referral form is fully completed and sent to the relevant personnel.
