Transition planning for children with SEBD

Ealing ran a Key Stage 2-3 Transition Project for Children with SEBD for several years. Colleagues who worked on the project developed a booklet containing guidance and relevant planning documents.

This advice continues to be a useful guide for good transition practice to support vulnerable students in Y6 as they move to Y7.

Please note that in order to keep in line with the timescales set out in the booklet, primary school colleagues should complete the 'Step 1' plan, parts 1-6 internally (ie without calling a meeting with the child and parents/carers). This will provide valuable information to the receiving high school. 'Step 2', the joint primary and high school meeting, should then be able to go ahead without delay.

The Primary Behaviour Service and the Educational Psychology Service team members will be available to support schools with this process. Schools should invite relevant professionals directly to the meetings.
