Supporting successful transitions

The aim of the project is to improve the primary to secondary transition experience for children with additional needs (including pupils with an ECHP and SEN Support).

The project is funded by the John Lyon’s Charity and is a partnership between two high schools, three primary schools and Ealing Council. It started in April 2023 and aims to develop and trial innovative ways to improve the transition experience for pupils, focusing on pupils and parents from Year 5 to the end of Year 7.

During the first year the focus is on:

  • Establishing the steering group, gathering baseline data, setting up systems to track progress and developing approaches for consulting with pupils and parents
  • Working with the Holiday and Food (HAF) programme to set up transition holiday programmes during Easter and Summer holidays, benefiting at least 40 young people
  • Developing and delivering transition workshops for Year 6 pupils and their parents
  • Developing and delivering parent workshops designed to empower parents to support their children through the transition process and develop strong, effective partnerships with schools
